Once you have applied for and received the Lottery and Gaming Credit, you need not file another application until your primary residence changes.
You may claim a lottery and gaming credit for a tax year if, on January 1 of the tax year, you were an owner of the property and used that property as your primary residence. (NOTE: "primary residence" is defined below)
If you meet these requirements and are NOT on the listing to follow, you may claim the lottery and gaming credit by going to either the County Treasurer’s Office, your municipal treasurer’s office or online and completing an application form. This completed form must then be returned to:

DO NOT APPLY IF YOU DID NOT OWN THE PROPERTY ON JANUARY 1 OF THE TAX YEAR, OR IF THE PROPERTY WAS NOT YOUR PRIMARY RESIDENCE AS OF JANUARY 1 OF THE TAX YEAR. THE WISCONSIN DEPARTMENT OF REVENUE WILL AUDIT ALL CLAIMS.
You may remove an item from this list if it was not your primary residence by picking up and completing a removal form at the County Treasurer’s office, your local municipal building or online and returning it to the County Treasurer’s Office.

Note: your primary residence is where you live most of the time. When you are temporarily away, it is the home to which you intend to return. You can have only one primary residence,

If your name appears on the listing on this web site, and you meet the eligibility requirements discussed earlier, you don’t have to do anything. Your name on this list means that you are pre-certified and you will automatically receive the credit on your tax bill.
If you have friends or relatives that do not have access to this listing via the internet, especially if they are elderly, please check for them and notify them if there is a need to make changes. THE PROPERTY OWNER MUST MAKE ANY CHANGES.
