Clinic Information - Immunization Services Starting January 2012

Vaccines are available for Diphtheria, Tetanus, Pertussis (Whooping Cough), Polio, Measles, Mumps, and Rubella, Hib, Hepatitis B, Varicella (Chicken Pox), Pneumococcal, Meningococcal, HPV, Rotavirus, and Hepatitis A.

Immunization Services Schedule
Effective January 2012
Main Office 8600 Sheridan Road Kenosha, WI 53143	Monday - Friday	7:00 a.m. - 6:00 p.m.

Mondays Tuesdays Wednesdays Fridays	8:00 a.m. - 12:00 p.m. & 1:00 p.m. - 4:30 p.m. 1:00 p.m. - 6:00 p.m. 7:00 a.m. - 12:00 p.m. 8:00 a.m. - 12:00 p.m.

THIS SCHEDULE WILL BE FOLLOWED EACH MONTH

Children must be accompanied by a parent or legal guardian. If someone other than a parent or legal guardian is bringing a child under 18 years of age to the clinic, the Immunization Permission (English) (Spanish) form AND Vaccine Administration Record English (Spanish) must be filled out by the parent ahead of time.

PLEASE BRING ANY RECORDS OF YOUR CHILD'S IMMUNIZATIONS.
NO RECORDS - NO IMMUNIZATIONS.

An $12.00 administration fee per immunization is requested.
