This document is part of the 2010 revision to the Department of Health Services Audit Guide, which is an appendix to the State Single Audit Guidelines for programs from the Department of Health Services. This information is to be used in conjunction with other sections of the DHS Audit Guide and with the Main Document of the State Single Audit Guidelines (available at www.ssag.state.wi.us)
Civil Rights Compliance Requirements
All Kenosha County Department of Human Services contracted providers are required to complete the Civil Rights Compliance-Letter of Assurance for the compliance period of 2010-2013, regardless of the number of employees and the amount of funding received. Providers with 25 or more employees AND who receive over $25,000 in funding must ALSO complete a Civil Rights Compliance-CRC Plan.

Outcome Based Evaluation (OBE):
KCDHS uses an Outcome Based Evaluation Approach to enhance program monitoring and evaluation practices. During a Request for Proposal process, proposal responses may require use of standard forms (logic model and program evaluation forms), followed by standard reporting methods for any resulting contracts.

Innovations Network: Logic Model and Evaluation Plan Workbooks
This site has excellent online logic model and evaluation plan "builders" along with easy-to-read, straight-forward handbooks. Requires registration (free).
UW-Extension: Program Development and Evaluation
The Cooperative Extension web site contains evaluation publications on topics such as logic models, data collection, program development, surveys, assessment instruments, and data analysis. There are also interactive, self-instructional web modules.
MAP for Nonprofits - Evaluation Activities in Organizations
MAP for Nonprofits provides management and board recruitment services to large and small nonprofit organizations in the Twin Cities. The site provides a wealth of practical information regarding evaluation in general. Resources found on this site are free.
United Way of America: Outcome Measurement Resource Network
The Resource Network offers information, downloadable documents, and links to resources related to the identification and measurement of program and community-level outcomes.
Child Welfare Information Gateway: Evaluation Toolkit and Logic Model Builder
An Evaluation Toolkit, developed by the FRIENDS National Resource Center, was created to assist child abuse and neglect prevention, family support, and parenting programs in evaluating the effectiveness of their programs in improving outcomes for children and families they serve. One component of the Evaluation Toolkit is the Logic Model Builder.
